Ignite Participation Waive *

I am the parent or legal guardian of the player I am signing up. I attest that I am eighteen (18) years old or older and that my child is physically fit and has no known medical conditions, which prohibit participation in this sport. I am aware that participation in the Ignite Hoops program has some inherent risks and injury can occur. On rare occasions these injuries can be serious. My child and I agree to follow all laws, rules and guidelines regulating the conduct of Ignite teams, training and camps. In consideration of my child being allowed to participate in the Ignite Hoops program, I, the parent/guardian, assume the risk of all injury and agree not to sue Ignite Hoops, the camp directors, coaches, assistant coaches, board of directors, or volunteers for any and all injuries caused by or resulting from participating in the Ignite Hoops program. By signing this waiver, I also authorize the use of photos, video and audio of the above-named participant to be posted on the Ignite Hoops website or advertising media published by Ignite Hoops. COVID - 19: I hereby agree to hold harmless Ignite Hoops and its officers, directors, agents, and representatives, of and from any claims arising out of my, my children and/or my family’s exposure to COVID-19, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Ignite Hoops and its officers, directors, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in Ignite Hoops sponsored activities.
